Has routine maintenance been kept up?

Hesitation at 2-3k rpm Under Load
Hello fellow xterra owners! Having a problem with my 05 SE 200k miles, when car is completely warm and I accelerate between 2-3k rpm it hesitates and the engine feels like it’s missing. I have no check engine light, engine temp is around 197-203 when this happens, trans temp is normal around 160-180
(bypassed the trans cooler to prevent smod years ago)
I do notice the battery volts fluctuate about .5 volts.
The vehicle also feels a little sluggish when this occurs.

I did valve covers myself about 2 weeks ago, did all plugs and an oil change, I also smoke tested it yesterday to see if there was any leaks but there was none.Has routine maintenance been kept up?

I have a bully dog tuner so I can read some stuff.
