For those that found it hard to get the center bolt for the leafs to lock the axle into place after the shim in place.
I modified 4 washers for each side to fit between the shim and push the center bolt up. It is very important that you use the highest grade washers you can find. I used 8 Hardened Grade 8 Washers. See pics below.
Center Bolt does not go past shim, so it cannot lock the axle in place.
Used a Dremel with a thin cutting disc.
Wear thicker gloves than I did because the hot metal flakes melted through mine and burned.
Fixed!!!
